Wrinkle treatment with botulinum toxin type A and hyaluronic acid
Although wrinkles are a natural part of skin ageing, they do not have to be accepted as inevitable – a view that is also shared by more and more men. Depending on the cause of the wrinkles, there are several minimally invasive treatment options available. Expression lines can be treated very effectively with botulinum toxin type A. Wrinkles caused by gravity and loss of volume can be corrected optimally with hyaluronic acid.
Wrinkle treatment with botulinum toxin type A
Botulinum toxin type A is a safe medication that has been used in neurology for over 40 years, even in young children. In cosmetic medicine, it smooths wrinkles caused by facial muscle activity. The full effect is achieved after 7 to 14 days.
Botulinum toxin type A is ideal for treating:
- Frown lines
- Horizontal forehead lines
- Crow’s feet
- Nose wrinkles
- So-called gummy smile
- Upper lip (smoker’s) wrinkles
- Chin wrinkles (cobblestone chin)
- Chin-lip wrinkles
- Vertical wrinkles on the neck
- Low-set, lateral eyebrows (eyebrow lift)

Wrinkle treatment with hyaluronic acid
Dermal fillers, such as naturally occurring hyaluronic acid, are gel preparations that are injected into the middle and deep layers of the skin or into the subcutaneous tissue. This smooths out deep, fixed wrinkles and compensates for the age-related loss of volume. The effect is immediately visible and lasts for approximately 9 to 12 months, depending on the product used, individual predisposition and lifestyle. In addition, the injected hyaluronic acid can stimulate new collagen production in the body.
Subcision can be a useful addition for particularly pronounced wrinkles that are fixed in the deeper layers of the skin by connective tissue strands. In some treatment areas, combining subcision with botulinum toxin type A can complement the effects and prolong the duration of the hyaluronic acid. In this case, the botulinum toxin type A treatment is carried out 14 days before the hyaluronic acid application.
Hyaluronic acids are an excellent solution for correcting:
- Nose-lip wrinkles
- Chin-lip wrinkles
- Drooping corners of the mouth
- Thin lips
- Nose contours
- Chin contours
- Sunken temples
- Sunken cheekbones
- Sunken earlobes
- Sunken cheeks
- Sunken hands
- Eyebrows
It goes without saying that Dr Sascha Dörrmann uses only organic, fully biodegradable filling materials.
Treatment examples
Botulinum toxin type A
Glabellar lines – the technical term for the vertical wrinkles between the eyebrows – and drooping corners of the mouth can be particularly distressing for those affected.
It is not without reason that they are also known as frown lines. Even when the face is completely relaxed, they can make a person appear grim and tense.
Botulinum toxin type A is a protein produced by bacteria and then highly purified. It has been used successfully in a wide variety of medical fields for decades. In aesthetic medical treatments, botulinum toxin type A reduces or eliminates unsightly expression lines and also has an excellent effect on the sweat glands of the forehead, armpits, hands and feet in treatments for excessive sweating.
How botulinum toxin type A works
Botulinum toxin type A is injected with extreme precision into the facial muscles that cause wrinkles. This inhibits the release of the neurotransmitter acetylcholine in the associated nerves and thus the nerve impulses that stimulate the muscles. The activity of the treated facial muscles is reduced, the corresponding areas of the face are relaxed and the wrinkles are smoothed out. In addition, botulinum toxin type A can prevent existing wrinkles from deepening and new wrinkles from forming. The effect lasts for an average of 4 to sometimes over 6 months.
How quickly botulinum toxin type A works
You will see the first results just a few days after the treatment. You can expect the full effect after 7 to 14 days. Dr Sascha Dörrmann will take this into account when planning your treatment(s) with you.
How painful is treatment with botulinum toxin type A?
All botulinum toxin type A treatments are performed with the smallest needles (0.3 mm). This makes the injection comparable to a mosquito bite. When applied to the face, a slight feeling of pressure is felt for a short time afterwards. Only treatments for excessive sweating on the hands and feet are relatively painful and are therefore performed under local anaesthetic or twilight sleep.
What else you should know about botulinum toxin type A
The treatment is generally well tolerated. Possible side effects will be discussed with you individually and in detail before each treatment. These are usually minor and short-lived. Short-term redness or, in rare cases, a small bruise at the injection site, as well as temporary headaches, may occur. Botulinum toxin type A only has a local effect and is completely broken down by the body over time. No undesirable long-term effects have been reported in the more than 40-year history of botulinum toxin type A treatment. Based on current knowledge, treatment with botulinum toxin type A can be repeated as often as desired. However, after repeated administration, antibodies may form in rare cases, which neutralise botulinum toxin type A and render it ineffective.
You should refrain from treatment during pregnancy and breastfeeding. The treatment is also contraindicated if you suffer from a chronic muscle/nerve disorder (e.g. myasthenia gravis, Lambert-Eaton syndrome or ALS), are allergic to any of the ingredients or are taking certain medications (e.g. aminoglycoside antibiotics, spectinomycin, aminoquinolines, cyclosporine, D-penicillamine, calcium antagonists).
Other areas of application for botulinum toxin type A:
Migraine and headache therapy:
For more than 10 years, botulinum toxin type A has also been used to treat tension headaches and migraines, taking into account specific pain acupuncture points.
Hair pain/scalp pain therapy (trichodynia):
Botulinum toxin type A is a new, highly successful therapy for hair pain (trichodynia) as well as scalp pain, itching, tingling and burning.
Night-time teeth grinding (bruxism):
Botulinum toxin type A is also used successfully to treat night-time teeth grinding and the excessive development of the chewing muscles (masseter hypertrophy) that is often associated with it.
Treatment of unpleasant, disruptive or excessive sweating (hyperhidrosis):
- Forehead
- Armpits
- Hands
- Feet
Dermal fillers (e.g. hyaluronic acid)
The term ‘dermal’ comes from the Greek word ‘derma’ (= skin). “Filler” comes from the English word ‘to fill’ (= to fill up).
Permanent and non-permanent dermal fillers
Fillers containing plastic components cannot be broken down by the body and therefore have a lasting effect, which is why they are also referred to as ‘permanent fillers’. However, due to numerous side effects, they are no longer used in modern, responsible aesthetic medicine.
Non-permanent (temporary) and natural dermal fillers, such as hyaluronic acid, have become established due to their excellent effectiveness and tolerability. These are completely broken down and metabolised by the body over time. It goes without saying that Dr Sascha Dörrmann only uses completely biodegradable, natural fillers.
Hyaluronic acid
Hyaluronic acid is an important natural component of connective tissue and can store many times its own weight in water. With increasing age, the concentration of hyaluronic acid in the skin decreases significantly. The consequences: the connective tissue stores less water, the skin loses volume and elasticity, and wrinkles form. The hyaluronic acid injected into the middle to deep layers of the skin or into the subcutaneous tissue for wrinkle treatment is cross-linked and is therefore only slowly broken down by the body. The latest generations of hyaluronic acid fillers create particularly smooth transitions between treated and untreated areas of the skin. In addition to cross-linked hyaluronic acid products, there are also non-cross-linked hyaluronic acid products. These are injected into the uppermost layers of the skin in combination with other active ingredients. By binding water, improving nutrition and stimulating collagen formation, they tighten and revitalise the skin (skin rejuvenation through meso-lift).
Dermal fillers are used here
Dermal fillers are particularly suitable for injecting deep wrinkles such as the nose-lip fold (nasolabial fold) and the chin-lip fold (mentolabial fold) or for treating fixed wrinkles and fine lines, for example on the cheeks or around the lips and chin. Hyaluronic acid is also used very successfully to improve the shape and add volume to the face and hands.

How quickly dermal fillers work
You can enjoy the effects immediately once the skin irritation caused by the injections has subsided.
What else you should know about dermal fillers (hyaluronic acid)
The treatment is generally well tolerated. Possible side effects will be discussed with you individually and in detail before each treatment. These are usually minor and short-lived. Redness and slight swelling at the injection site, as well as slight, small bruises may occur. You should refrain from treatment during pregnancy and breastfeeding.
Subcision
In order to achieve optimal results, it may be necessary to treat connective tissue strands in the deeper skin layers before applying the filler in cases of particularly pronounced wrinkles. This improves the lifting power of the filler during the subsequent wrinkle injection and achieves a longer-lasting result.
